All right, the skull puzzle first. I think you are talking about that part with the wheels and a four by four grid? If so, it works something like this: Each wheel will turn its row or column. It will not affect any of the others. What I would do to solve this is first roll the side ones to get a skulless vertical corrider. If one of them will not work, then run down the bottom wheels turning each of them once, then try the row you were walking on again. Repeat until you can reach that powerup on the side with the wheels. Then, it should be simple to roll that skulless corrider over to the other side. Just hit all the row switches once. For the vampire doll, just keep trying. As for the lake, be a bit more specific, deeper under the lake or just deep under the lake. That skull key you can't find is under the lake... keep looking.

Yes, as far as the dolls go, there's a random chance of one appearing each time you kill the appropriate type of monster, so sometimes it happens on the very first guy (very frustrating as a game designer...), sometimes it takes forever! Just keep on playing and if it hasn't happened by the time you're at the end, THEN you can start worrying about it. You do get to keep playing after you win, by the way, but you only get "graded" once - and that's when you finish the game. So it's best to have all the quests done at that point if you want a really good score. And not to give anything away, but you DO.

Those fuses control the power to two things, the doors and the electric gates. Each switch corresponds to one segment of the area, the upper right fuse affects the upper right area of the whole place. The trick is to get to the center, there you can go down another level. If an area does not have power you may pass the gates, but the doors won't work. The opposite is true for powered sections. Before you leave you might want to check out the area for a special weapon, my personal favorite is hidden down there.
