I have just completed mad cap mode and it has took me back to mad cap mode again. I have all the fairies, but reading other posts I think I am missing Insane Rage.
Can anyone help me find it..

It was in the vault...
I am convinced that it wasnt there the 2nd time I went through this level...
On challenge mode now..
