On subsequent play throughs does every count continue?

When ever you start a new game, does the count of enemies killed, etc., start over from the beginning, or keep going from what you had before. What about achievements? Can you only get them by continuing in madcap mode, or will just replaying in any mode work?

I'm not sure about enemy counts, but if you're going for one of the enemy killing achievements, you should definitely do it pre-Madcap while it's much easier.

It doesn't matter if you go into Madcap- All your currently achieved achievements stay the same, and you can play in any mode to get achievements (including Madcap).
