Questions on Achievements

To get the 4 hour achievement does it have to be a new game or can it be a restart of madcap, also can you just complete all quests or does it have to be 100% including opening all chests?

You have to kill the happy-stick-bird(BOSS) in under 4 hours. Then the normal mode is finished.

It can be a restart of Madcap, but it's easier in normal.
